Camper Application Questions

Before you begin your online application for a camper, you will need to know the following information to complete the application:

General Camper Information

• Name, Home address, Gender, Birthday, Grade, T-Shirt Size

Camper Background Information

• Swimming ability, English fluency, Armenian fluency
• Church, Sunday School, and Armenian Day School attendance
• Name, age, and grade of any cabin roomates they wish to request

Parent/Guardian Information

• Name, addresses, Medial phone numbers, emails
• Emergency contact information

Camper Medical Information

• Doctor's contact information (name, phone number)
• Dentist's contact information (name, phone number)
• Medical Insurance Information (company name, phone number, policy, group #)
• Allergies
• Immunization History (you need to know if they have been immunized for the following)
  
  • Diptheria, tetanus, pertussis
  • Tetanus booster
  • Mumps, measles, rubella (MMR)
  • Polio (IPV)
  • Haemophilus influenzae B (HIB)
  • Pneumococcal
  • Hepatitis A
  • Hepatitis B
  • Varicella (Chicken Pox) - Date of when they had it if they have
  • Meningococcal Meningitis (MCV4)
  • Tuberculosis (TB) test - most recent date

• List any medication camper is taking
• Any medications you do not want administered to your child
• Health history (any major items of note)

Transportation to Camp

• You should know how your child will be getting to camp (i.e. Bus from South, Central, or Northern California OR You will be driving them)
